What is PsyCoXx?

PsyCoXx is a computer-adaptive cognitive assessment tool being developed for clinical use in memory assessment pathways. It uses patented technology based on Multidimensional Item Response Theory (MIRT) to deliver a single assessment covering three domains: cognition, functional ability (activities of daily living), and depressive symptoms.

Rather than using a fixed set of questions, PsyCoXx is designed to adapt in real time to the individual's responses — selecting only the most informative questions for that person's ability level. This means no irrelevant or inappropriately difficult questions, and a complete scored output even if not every item is administered.

The assessment scores each of the three domains separately for both the person being assessed and a carer, friend, or family member — combining both perspectives into a more complete clinical picture.

Limitations of current approaches

Cognitive assessments are a routine and essential part of memory clinic evaluation. However, widely used tools such as the MoCA, ACE-III, and MMSE have well-known limitations:

Common challenges

  • Fixed-length — every person receives the same questions regardless of ability, including items that may be far too easy or too difficult
  • Limited flexibility for incomplete assessments — some cognitive tools rely on completion of all tasks to generate standardized scores, making results harder to interpret when items are missed
  • Cognition-focused — most widely used cognitive screening and neuropsychological batteries do not comprehensively assess functional ability and depressive symptoms, necessitating supplementary instruments in many clinical and research settings

What this means in practice

  • People with significant impairment may be asked questions that are distressing and uninformative
  • Assessment time is spent on items that add little diagnostic value
  • Clinicians must combine multiple separate tools to get a rounded picture, increasing burden on both clinic and individual
  • Sensitivity to change over time can be limited by floor and ceiling effects
Important: No single cognitive assessment can diagnose dementia on its own — results must be interpreted alongside clinical history, examination and other investigations as appropriate. PsyCoXx is being designed to support, not replace, clinical judgement.

The three-domain approach

What does PsyCoXx measure?

Unlike purely cognitive test batteries, PsyCoXx is designed to assess three distinct domains in a single sitting — providing what we call a "3D view" of the individual. This removes the need to administer separate instruments for each area.

Three domains, two perspectives

  • Cognition — a single adaptive score for cognition overall, with the MIRT algorithm selecting items matched to the individual's ability level
  • Functional ability (ADL) — questions about activities of daily living to assess real-world impact of cognitive changes
  • Depressive symptoms — mood indicators that can interact with and mimic cognitive impairment, included to support differential interpretation

How is PsyCoXx designed to work?

PsyCoXx is built on a patented MIRT-based Computer Adaptive Testing (CAT) engine. The algorithm estimates the person's ability in real time and selects the next question based on which item would provide the most information at that ability level. This means the assessment skips items that are too easy, too hard, or unlikely to change the score — keeping the experience focused and efficient, typically within 15–30 minutes.

The tool runs in any standard web browser — no app installation or specialist hardware required.
